By entering the Ben Machine facility and completing the form below, I agree with the following:

OCCUPATIONAL HEALTH AND SAFETY

I will not operate any machines without obtaining a safety briefing from Ben Machine’s safety officer. I will not perform any work or repair any equipment without being licensed and bonded in accordance with the requirements of my applicable trade for the relevant equipment. I will not perform any work without appropriate personal protective equipment, including safety shoes, safety glasses and hearing protection. I will not use a ladder over three metres unless the ladder is supported by a second person and complies with all Occupational Health and Safety Act regulations. I will not access spaces over three metres without using a safety harness to secure myself from a fall. I acknowledge that Ben Machine can supply a safety harness for such purposes. I will comply with all requirements of the Occupational Health and Safety Act applicable to me. If I am unclear about any safety requirements, I must request information from Ben Machine’s safety officer.

CONTROLLED GOODS REGULATIONS

I will be given a security badge that I agree to wear at all times while on Ben Machine’s premises. I will remain at all times with a Ben Machine employee while on the premises. I will not access or view any documentation or goods other than those originated from my company. I will treat all information, documentation and goods accessed at Ben Machine as confidential and will not disclose such information to anyone for any purpose. I will immediately advise Ben Machine if I am not a Canadian citizen or a permanent resident ordinarily resident in Canada. In such circumstances, I will not be able to visit any areas of the facility outside of the front boardroom and lobby without an approved application for exemption for visitor form from the Canadian government prior to entry.
